Diabetes TARGET – North & East – 8 June 2022

Our first TARGET session returning face-to-face! Please note that lunch will be available from 12pm. Should you have any dietary requirements, please complete on the booking form.

When you arrive at the venue, please have your QR code either on your phone or printed out, to be checked in.

This session will be compiled with many different breakout sessions, so that clinicians can personalise their learning.

The breakout sessions are:

  • Language matters – Professor Heather Daly
  • Complex patient – Yassir Javid
  • Diabetic eye screening – DBTH team

Meet the speakers!

Dr Yassir Javaid

Cardiovascular lead Nene CCG, Primary Care Cardiovascular lead, East Midlands Clinical Network

MBBChir, MA, MRCP, MRCGP, DRCOG, DipCard

Dr Javaid qualified from Cambridge University and completed his GP VTS training in Northampton. He has an interest in cardiology and echocardiography and was a clinical lead in the Northamptonshire Community Cardiology service, which had a focus on patients with heart failure and valve disease.

He was named Pulse “GP of the Year” in 2015 for his work in reducing stroke emergency admissions in the East Midlands. He is also a council member of the British Heart Valve Society, accredited member of the British Society of Echocardiography and on the editorial board for the British Journal of Cardiology.

Professor Heather Daly

Heather has worked as a Nurse Consultant at University Hospitals of Leicester for past 10 years, working collaboratively with primary care trusts to improve diabetes services across
Leicestershire. The last 28 years of her nursing career have been spent working in the field of Diabetes. This was provisionally in Nottingham and Derby, and now in Leicester.

More recently she moved into a Nurse Consultant role in NIHR Collaborative Leadership in Applied Health Research and Care (CLAHRC). Her role is one of leadership, vision
and strategic management and she leads a diverse team of healthcare professionals and researchers from a variety of backgrounds. Heather’s research interests are in gaining
research funds to develop and test structured education programmes which support self management in people with diabetes, in the prevention of diabetes in those at risk, people
with diabetes and learning disabilities and for women with polycystic ovary syndrome (PCOS). She is currently working on programmes to address vascular risk such as heart disease and stroke, sleep apnoea and mental health. This includes the training and quality developments of healthcare professionals delivering these interventions. Heather has a national profile with the DESMOND programme and is a national expert trainer for ‘Conversation Maps’. She has enjoyed being instrumental in developing and establishing the nationally recognised Diabetes Masters Programme with the University of Leicester and has a keen interest in healthcare professional (HCP) education to improve standards of care for people with diabetes.

Jane Diggle

Specialist Diabetes Nurse Practitioner, College Lane Surgery, Ackworth, West Yorkshire Jane qualified with a BSc (Hons) in Nursing in 1990 (Leeds) and has over 20 years’ experience working as a practice nurse. She recently took up a new post specialising solely in diabetes within a general practice setting.

Dr Matt Capehorn

Matt Capehorn founded and is a Clinical Manager and GPwSI at the Rotherham Institute for Obesity (RIO). RIO is a part of the award-winning NHS Rotherham Weight Management Strategy that won the 2009 NHS Health and Social Care Award for best commissioned service.

Louise Dodd, RGN, MSc, INP

Louise joined Lilly after a thirty-year career in nursing, which focused upon primary care. Her career began in district and community nursing, which she later combined with study for an MSc, University lecturing and research. Louise has a passion for diabetes care in the community and held the role of Diabetes Specialist Nurse for twenty years. She is an expert in the development of diabetes services in local communities, contributing directly to the prevention of diabetes and the reduction of hospital admission. She is a thought leader in diabetes care, actively involved in education and mentoring.
